Semaglutide with B12: what it does and doesn’t do
Why it’s there. Some pharmacies add vitamin B12 to semaglutide. They don’t all say why.
What’s known. A 2026 review of compounded GLP-1 products found added ingredients in about half, and limited evidence for adding vitamins without a documented deficiency.
What it changes. The color: B12 turns the liquid red or pink. Not the semaglutide strength on your label.
What the FDA says. In its example, a GLP-1 combined with B12 can still count as essentially a copy of an approved drug when both amounts are within 10% of approved strengths.

Dose in milligrams, not units. At 2.5 mg per mL, 10 units (0.1 mL) is 0.25 mg of semaglutide; at 5 mg per mL, the same 10 units is 0.5 mg. If the strength on a new vial is different, don’t guess. Call or text us before your next dose.

What is semaglutide sodium?
A salt form of semaglutide. The FDA has warned about compounded products made with salt forms like it.
Is semaglutide acetate the same as semaglutide?
No. It’s a salt form, and the FDA has raised the same concern about it as about semaglutide sodium.
Does compounded semaglutide have B12?
Some formulations do. B12 turns the liquid red or pink, and your label lists it when it’s there.
Is compounded semaglutide the same as Ozempic or Wegovy?
No. It isn’t FDA-approved, and its formula can differ by pharmacy. Wegovy and Ozempic each have one approved formula, made by Novo Nordisk.
What preservative is in compounded semaglutide?
Multiple-dose vials usually contain one; your label names it. Wegovy’s single-dose pens don’t need one.
Why is my semaglutide pink?
Most likely added vitamin B12. Color doesn’t tell you the strength; your label does.

Important safety information
Compounded tirzepatide and semaglutide are not FDA-approved, and the FDA hasn’t reviewed them for safety, effectiveness or quality. Both medicines may cause thyroid tumors, including cancer: don’t use either if you or a family member has had medullary thyroid carcinoma (MTC) or multiple endocrine neoplasia syndrome type 2 (MEN 2).

In studies with rodents, both medicines caused thyroid tumors; it isn’t known whether they cause them in people. Tell your provider if you notice a lump or swelling in your neck, hoarseness, trouble swallowing or shortness of breath.
Serious side effects can include inflammation of the pancreas (pancreatitis), gallbladder problems, low blood sugar (especially with insulin or sulfonylureas), kidney problems from dehydration, severe stomach problems, serious allergic reactions, vision changes in people with type 2 diabetes, and depression or thoughts of suicide. Tell your care team before any surgery or procedure that uses anesthesia or deep sedation. Don’t combine either medicine with other GLP-1 medicines.
Tirzepatide can make birth control pills less effective: use another method for 4 weeks after you start and after each dose increase. Semaglutide should be stopped at least 2 months before a planned pregnancy. Tell your provider if you’re pregnant, planning to be, or breastfeeding.
